Initial Consultation: Schedule a FREE virtual or in-person consult to discuss your health concerns, history, and goals.

  • Assessment & Enrollment: Review your needs; if it's a fit, choose a plan (Prime for self-pay or Prime PPO with insurance) and complete quick setup.
  • Personalized Care Plan: Your provider develops a custom plan focused on wellness, prevention, and any immediate issues.
  • Ongoing Access & Monitoring: Unlimited visits (virtual/in-person), 24/7 support, regular check-ins, and adjustments for optimal health.

Our Concierge Primary Care is affordable with transparent monthly fees—no co-pays or deductibles for included services. Prime is self-pay for full independence; Prime PPO offers reduced pricing when paired with your PPO insurance (we must accept it). Discounts on labs, X-rays, and supplies (85% of retail) add value. Optional HRT-Wellness add-on for hormone optimization. This is not insurance—it's enhanced primary care.

Concierge Medicine Membership Program Disclaimer

Please note: This is not health insurance and is not a substitute for health insurance or other health plan coverage (such as membership in a health maintenance organization). Concierge Primary Care covers traditional primary care medical services but does not include emergency room visits, hospitalizations, or consultations with medical specialists. Labs, X-Rays, and Medical Supplies are charged at Concierge Member pricing, on average 85% of retail pricing. We recommend maintaining insurance for comprehensive coverage, as additional costs may be incurred for laboratory, medical imaging, surgery, specialist care, emergency department visits, and hospitalization required outside of our services. No Guarantees: While we strive to provide high-quality, accessible care, no specific health outcomes, results, or benefits are guaranteed. Medical care involves inherent uncertainties, and individual responses to treatments vary. Services provided are based on professional judgment, but success depends on factors beyond our control, such as patient adherence and underlying conditions. Pre-existing conditions do not disqualify enrollment, but we make no assurances about resolving them. Services and pricing are subject to change at our discretion without prior notice.

Medication Management: Prescription management and refills are included for covered services, with efforts to optimize access and cost (e.g., in-house options or competitive pricing guidance). However, we do not guarantee medication availability, efficacy, or absence of side effects, interactions, or allergic reactions. Medications are prescribed based on clinical judgment, but ultimate responsibility for use lies with the patient. Consult your provider for any concerns; we disclaim liability for decisions related to high-deductible plans or HSAs that may affect contributions—seek advice from your financial advisor.

Risks & Limitations: Participation involves general medical risks, including potential misdiagnosis, treatment complications, or delayed care if not using appropriate channels (e.g., emergencies require 911, not our services). Digital communications (email/text) carry risks such as interception, misaddressing, unauthorized access, alteration, or breaches by third parties; they are not secure for sensitive information (e.g., HIV, mental health) or emergencies. Non-covered services (e.g., hospital stays, advanced imaging, specialists) require separate payment or insurance billing, with possible assignment of benefits to us. Limitations include: no coverage for major events like surgeries or ambulances; virtual care is not suitable for physical exams or acute issues; potential termination for non-payment, misconduct, or at our discretion, with 30-day notice. Fees are non-refundable, and declined payments may incur $25 fee.

General Protections: We prioritize your privacy under HIPAA, with safeguards for protected health information (PHI), including secure systems and staff training. You control your information; we may share it with your health plan for care coordination (e.g., visits, prescriptions, biometrics) as permitted. Informed consent is required for all services; onboarding includes HIPAA authorizations and notices. For disputes, this agreement is governed by California law, with binding arbitration under the Federal Arbitration Act—no jury trials. We are not liable for breaches caused by you or third parties, or for improper digital disclosures not due to our misconduct.
